Please read the instructions and each question thoroughly to provide complete, accurate information. Please let us know if you have any questions or problems in using the online registration. While it has been tested, keep in mind there may be issues we haven't yet discovered. Therefore, we ask for grace as we "go live" and your patience and forgiveness if difficulties are encountered!
While On-line Registration eliminates the need for paper registration, there are two forms that require a signature. At the present time we do not have the capability within our form to accept an electronic signature, so these documents will have to be downloaded signed and/or notarized, then uploaded, mailed or delivered to SPLASH.
